There is a $5.00 charge for each disability form our staff fills out.  If you have these forms, bring them to the office and give them to the front desk staff.   Make sure your full name and your contact information is with the forms.   Please fill out the portion that applies to you before bringing the form to the office.

Our office will notify you when these forms are ready unless there are instructions to either mail the forms elsewhere or fax them.
